Please take time to review this information carefully. Completing the attached survey enrolls you in a research study. Your participation in this study is voluntary which means that you may choose not to participate.

If you decide to take part in the study, you will be asked to sign the consent check box on the previous webpage, be sure you understand what you will do and any possible risks or benefits.

 

Key Information

You are being asked to participate or be a part of a research study because you completed the Game of Telephone installation experience. Your participation is voluntary which means that you may choose not to participate at any time. Please read this form and ask any questions before you agree to be in the study. If you decide to take part in the study, you will be asked to sign this form. Be sure you understand what you will do and any possible risks or benefits.

 

Our primary focus at this stage is on better understanding how to design a fun and rewarding experience for the user. The researchers hope to learn more about your experience with the telephone booth game to learn how for future modifications to the experience. You are being asked to participate because you completed the experience and have navigated to this survey on the project website. This study is only intended for people who are 18 years of age or older. Do not complete the study if you are not legally considered an adult.

 

What will I do if I choose to be in this study?

If you choose to be in this study, you will be asked to answer survey/interview/focus group questions. You can skip questions that you do not want to answer or stop the survey/interview/focus group at any time.

 

The survey is anonymous.

 

This survey is intended to be anonymous, so we do not plan to collect your name or information that will directly link your answers back to you. Please do not include your name or other information that could be used to identify you in the survey responses.

How long will I be in the study?

​

We expect that the survey/interview/focus group will take about five minutes. You will be asked to complete all answers you wish to answer. The study team plans to keep answers for this study to answer research questions. We will keep this information until we are done with the study, approximately two-years from the date of completion and for at least three years after we are finished. We may share the anonymous data and findings with other researchers or in research papers or presentations.

What are the possible risks or discomforts?

​

All research carries the risk of breach of confidentiality which means that someone outside of our study could figure out that you were in the study or information was yours. We will take steps to minimize that risk including by maintaining anonymity with your answers.

Some questions could make you feel uncomfortable. If this happens, you can skip any of these questions or stop answering. We anticipate that the risks of participating in this research are no greater than what you would encounter in daily life or during the performance of routine physical or psychological exams or tests.

Your personal information may be shared outside the research study if required by law. We also may need to share your research records with other groups for quality assurance or data analysis. These groups include the Purdue University Institutional Review Board or its designees, and state or federal agencies who may need to access the research records (as allowed by law).

Are there any potential benefits?

It is unlikely that there will be direct benefits to you for participating. Having more information from the answers in this research study might help us or other researchers understand more about designing interactive installation experiences.

Will I receive payment or other incentive?

You will not receive compensation for your participation in the study.

Will information about me and my participation be kept confidential?

The project's research records may be reviewed by the study sponsor/funding agency US DHHS Office for Human Research Protections, and by departments at Purdue University responsible for regulatory and research oversight. Survey data will be stored securely in a Purdue affiliated Box account until the end of the five-year period stated prior and deleted upon the end of that period. The aggregate data or volunteered responses may be used anonymously in research publications and/or for teaching purposes.

What are my rights if I take part in this study?

You do not have to participate in this research project. If you agree to participate, you may withdraw your participation at any time without penalty. Because this is an anonymous survey, it is possible that your answers will not be able to be distinguished from other responses after your participation. We will not be able to send you results or remove your answers at this point. Any decision not to participate will not affect your relationship with the university.
